Membership Infomation & Update

If you wish to become a member of our support group, please download the attached form, fill it out,  and send it to our treasurer, whose address is shown on the form.  Current members should also use that form to update their information, such as a new email address and mail it to our treasurer. We ask that members donate $20 a year to our group to help defray operating costs, such as postage, paper, and refreshments at meetings.  Your donation is tax deductible.  If you cannot afford that much, please send us whatever you can.  We will never remove anyone from our membership list because of an inability to make a donation.  We will ensure that any medical information that you provide (such as the type of dystonia) will be kept confidential.
